Service Agreement Client Service Agreement Between: HomeBridge Senior Placement Services (“Agency”) and Name . Effective Date: MM slash DD slash YYYY 1. Purpose This Agreement outlines the placement services provided by HomeBridge Senior Placement Services to assist the Client in locating and transitioning to appropriate senior housing or care options. 2. Services Provided The Agency agrees to: Conduct an assessment of the Client’s care needs, preferences, and financial situation. Provide customized recommendations for adult family homes, assisted living, memory care, or other suitable care settings. Schedule and coordinate tours of potential facilities. Support the Client throughout the decision-making and transition process. Follow up after placement to ensure the Client’s comfort and satisfaction. 3. Fees 3.1 All services provided by the Agency to the Client are free of charge. 3.2 The Agency receives compensation solely from the facility in the form of a referral fee when a placement occurs. 3.3 The Client is never responsible for any fees or payments to the Agency. 4. Confidentiality 4.1 The Agency will maintain strict confidentiality of all personal, medical, and financial information provided by the Client.4.2 Information will not be shared with any third party without the Client’s prior consent, except as required to facilitate placement. 5. No Guarantee 5.1 The Agency provides recommendations based on available information, but the final decision regarding placement rests solely with the Client and/or their legal representative. 5.2 The Agency does not guarantee acceptance by any facility or the long-term suitability of any placement. 6. Relationship of Parties The Agency acts solely as an independent consultant and does not own, manage, or operate any of the facilities to which clients may be referred. 7. Termination Either party may terminate this Agreement at any time by providing written notice. Termination does not affect any placements or referrals completed prior to notice. 8.
